The Doctor of Physical Therapy program is an entry-level professional course of study for individuals with an earned baccalaureate degree at an accredited college or university with a minimum grade point average (GPA) of 3.0 on a scale of 4.0 in which 4.0 is equivalent to an 鈥淎.鈥� Prerequisite courses must be completed with a 鈥淐鈥� or better grade in each course, and with an overall prerequisite GPA of 2.5. Prerequisite courses must have been completed within the past 10 years from the application deadline.听

To be considered for admittance to 麻豆社鈥檚 School of Physical Therapy and the DPT program, the following requirements must be met and submitted via the :听

• Submit a completed application for admission.听
• Submit official undergraduate and graduate transcripts from each institution attended. Submit final undergraduate transcript that confers baccalaureate degree.听听
• Complete the Prerequisite Courses & Requirements Completed form.听
• Provide three letters of recommendation.听
• Submit Graduate Record Examination (GRE) scores. Scores should be no older than five years prior to the application deadline.听
• Pay a non-refundable application fee of $75 for in-state applicants and $100 for out-of-state applicants. Payment can be mailed to 麻豆社, School of Physical Therapy, P.O. Box 1500, Langston, OK 73050 or
• Complete an autobiographical statement form.听
• Provide documentation of 50 hours of clinical observation.听
• Submit medical records听
  • Immunization history MUST INCLUDE
    • MMR vaccine (2 doses)听
    • Varicella 1 & 2 vaccine series or titer听
    • Hepatitis B vaccine series (3 doses)听
    • Current tuberculosis test (within one year of application deadline)听
